The article discusses the inauguration of the new facilities for the Unidade de Cuidados na Comunidade (UCC) Cruzeiro do Algueirão and Rio de Mouro in Mem Martins, Portugal. The UCC, which was established in December 2010, has moved from cramped quarters in the Centro de Saúde de Algueirão-Mem Martins to a renovated space that enhances its ability to provide healthcare services to the community. The UCC focuses on home care for patients with physical or psychological dependencies, health education, and support for vulnerable populations, including family planning and newborn care. It operates with a team of ten professionals, including doctors, nurses, and social workers, and currently assists 130 patients at home. The new facilities, which cost approximately 50,000 euros, aim to improve access to healthcare and support for local residents. The article highlights the importance of community-based healthcare and the collaboration with various partners to deliver comprehensive medical, social, and psychological care.
