Integrated Continuing Care Teams (Equipas de Cuidados Continuados Integrados or ECCI) are multidisciplinary teams that coordinate long-term, post-acute and rehabilitation care for patients who no longer need acute hospital treatment but still require medical and social support. A 43% rise in patients awaiting placement means hospitals hold patients longer, increasing pressure on beds, slowing discharges, and worsening access to acute care for other patients.
